Welcome to these pages for young people and teenagers, aged 12 to 18, who use our services to receive treatment for the blood disorders known as sickle cell disease and thalassemia. You can find out more information about your condition, as well as helpful advice about how to get the help you need with anything from general health matters to mental health support. You'll also find sample plans to help your transition to adult services. We know this can be a stressful time, so if there is anything else you would like to see on these pages, please let your transition coordinator know.
