About our Anticipatory Care Service

Our anticipatory care service is proactive healthcare and support, targeted at people of all ages living with frailty, multiple long-term conditions and/or complex needs to help them stay independent and healthy for as long as possible at home, in the place they call home or in their local community.  

The Anticipatory Care team has two Peer Workers, four Care Navigators, one Registered Mental Health Nurse and two Community Matrons to support with proactive identification of patients. The Anticipatory Care service sits under the umbrella of the Community Matron service. Patients can be sourced through MDT meetings where risk stratification takes place or referred directly from primary care services, secondary care or stakeholders. 

This service is available Monday to Friday, 9am - 5pm. If you would like to know more about this service, please email the MDT Coordinator, Androulla Christofi (androullachristofi@nhs.net) to arrange a suitable date/time for a presentation to take place.
