About our community matrons service

We provide treatment and support to people with a long term condition such as diabetes, cardiac and respiratory disease, who have needed recent emergency hospital treatment or who frequently consult their GP. 

Community matrons are senior, experienced nurses who are autonomous practitioners, with enhanced skills including independent prescribing, physical assessment  and advanced care planning to facilitate and promote independence in the community by proactive case management

What we offer

The aim of our work is to prevent unnecessary hospital admissions and improve patient outcomes for people living with life long medical conditions such as diabetes, cardiac and respiratory disease.

The care of patients with long term conditions, which include asthma, arthritis, diabetes, chronic obstructive pulmonary disease, renal disease and heart failure, accounts for around 80% of GP consultations, therefore the work of the Community Matron also assists GPs in managing their work load.

Community Matrons work closely with patients and their carers using an integrated approach with GPs, social services, hospital and voluntary services to improve the management of long term conditions for people in their own homes.
