Diabetes service in the community

We provide education, support and advice to people with diabetes, their carers and other health professionals. 

The team is led by a diabetes specialist nurse lead, diabetes specialist nurses, a diabetes dietician and the administration team.

What we offer

• We offer Type 2 diabetes education (Education App, Virtual and Face to face).
• Diabetes Consultation including medication review.
• Support and empower patients, their carers and other health professionals in the self-management of Type 2 Diabetes

How you can access this service

If you think that you could benefit from Type 2 diabetes education, support and advice please speak to your GP or Health Care Professional who can complete and send the diabetes e-referral form to beh-tr.ECSenfieldlocalityteamsSPA@nhs.net.

As soon as your referral form has been sent to us (Enfield Community Diabetes Service), we check all referrals daily, Monday to Friday), we will then decide which will be the most appropriate clinic(s) for you to attend. You may receive appointments for more than one clinic e.g diabetes specialist nurse and dietician clinic appointment.
