About our hospital at home for babies, children and young people

The Hospital @ Home team consists of highly trained and experienced paediatric nurses who provide care for babies, children and young people aged 0-16 (and 16-18 year-olds with complex needs) who live in Haringey and the N postcodes of Enfield.

The H@H team aim is to care for children who are acutely unwell in their homes instead of hospital and reducing or avoiding the need for hospital admission.

Conditions we treat

  • Neonates (postnatal parenteral antibiotics).
  • Respiratory (viral-induced wheeze, exacerbation of asthma, lower respiratory

tract infections, bronchiolitis, and croup)

  • Gastroenteritis
  • Febrile Illness
  • Neonatal Jaundice Management

How to prepare for your visit (what to expect)

Patients should expect to receive a phone call up to 4 hours after the referral has been made. In this call, the H@H nurse will do an initial triage and agree on a date and time of the patient's first visit. Any additional information will be provided by the nurse based on the patient’s needs.

Refer a patient

Referrals from North Middlesex University Hospital
Patients who are currently inpatients at NMUH can be referred to the H@H team by NMUH Clinical Staff. The staff member should contact the nurse in charge to discuss patient details and then complete a referral form. The clinical responsibility will remain with the attending consultant at NMUH.

Referrals from GPs, HCPs and Community
Patients who meet our inclusion criteria from other primary care services can be referred to the H@H team by the clinician. The referral process is to contact the nurse in charge to discuss the patient and then complete a referral form. The clinical responsibility will then be held by the attending paediatric consultant at NMUH and the referrer will be updated during the admission period and upon discharge.
Any prescription required at the point of admission are the referring GP’s responsibility. 

Referrals from Other Trusts
Patients who are currently inpatients at other trusts and meet our inclusion criteria can be referred to the Hospital @ Home team by Clinical Staff. The staff member should contact the H@H nurse in charge to discuss the patient’s needs and then complete a referral form upon the nurse accepting the referral verbally. The clinical responsibility will remain with the paediatric consultant from the referring team.
